Changes to pharmacists‘ details - notification form
Complete this form to meet your statutory obligations as a pharmacist to advise the Board of a change in residential, mailing and practice addresses


A pharmacy, pharmacy department or depot must be conducted in premises that have been approved by the Board.

Criteria for approval
The Board assesses applications for compliance with the planning requirements set out in the Pharmacy Board of Victoria Guidelines for Good Pharmaceutical Practice (Part 6 - Pharmacies and Pharmacy Departments).

To establish a new pharmacy department or relocate or alter an existing pharmacy department
Pharmacists should lodge an ‘Application for Approval of Pharmacy Department Premises‘

To establish a new pharmacy or relocate or alter an existing pharmacy
Pharmacists should lodge an ‘Application for Approval of Pharmacy Premises‘

To establish a pharmacy depot
Pharmacists should lodge an ‘Application for Approval of a Pharmacy Depot‘


A person must have the prior approval of the Board to:
- establish a new pharmacy business or pharmacy department, or
- buy an existing pharmacy business or a partnership in an existing pharmacy business, or
- relocate an existing pharmacy business, or
- assume control of a pharmacy department, or
- relocate an existing pharmacy department.

Section 107(1) of the Health Professions Registration Act 2005
A pharmacist shall not supply, compound or dispense except
(a) from a pharmacy or pharmacy department  that is approved by the Board (refer Approval of premises); or
(b) in any other special circumstances that are approved by the Board in a particular case.

Criteria for approval
The Board assesses applications on the basis of the public need and compliance with the relevant standards of practice

To obtain approval to supply, compound or dispense in special circumstances
Pharmacists should lodge a written application setting out the special circumstances (‘Application for Approval to Supply, Compound or Dispense in Special Circumstances pursuant to section 107(1)(b) of the Health Professions Registration Act 2005‘)


The Board must be notified of changes relating to the ownership of pharmacy businesses.

Notification of commencement to carry on a pharmacy business


Notification - cease to carry on a pharmacy business


Notification by a pharmacist of being appointed as a director or of acquiring shares in a company carrying on a pharmacy business

Notification by a company that is carrying on a pharmacy business of the appointment of a director or of the allocation of shares

Notification by a company that is carrying on a pharmacy business of the retirement of a director or of a shareholder disposing of shares
