An "Application for Provisional Registration" must be lodged in accordance with the procedures outlined on the application form which includes providing a suitable form of identification and evidence of qualification for provisional registration.  The Board may refuse to grant provisional registration or apply conditions to provisional registration in accordance with section 9 of the Act.  Provisional registration may be granted for a period not exceeding 24 months.

Graduates of non-Victorian pharmacy courses (other than APC candidates) are also required to submit a 'Consent to Disclosure' form.

Interim provisional registration is granted at the discretion of the Registrar or the Board, with or without conditions.  Click here for further information.
